We coordinate supports for participants right across Queensland. In South East Queensland we come to you in person; further out we work by phone and video, and travel when a situation needs someone in the room.
The Care Coordinators are based in Redland City in bayside Brisbane and provide NDIS support coordination in person across South East Queensland — Brisbane, the Redlands, Logan, Ipswich, Moreton Bay, the Gold Coast and the Sunshine Coast — and by phone or video anywhere else in Queensland. Home visits are standard, not an extra.

Distance changes how we work, not how much we do. Regional participants get the same coordinator, the same follow-through, and phone or video contact at times that suit — plus travel when something can only be sorted in person.

Questions about coverage
Do you actually come to my house?
In South East Queensland, yes — the first meeting is usually at home, or wherever you're most comfortable. Outside that, we'll set up video or phone and travel for the moments that need it, like a SIL house tour or a hospital discharge meeting.
Does travel come out of my plan?
Provider travel is claimable under the NDIS price limits, so it does draw on your Capacity Building budget. We tell you what a visit will cost before we make it, and we don't travel for something a phone call would have fixed.
I'm moving within Queensland — do I have to change coordinators?
No. We can keep working with you anywhere in the state, and a move is exactly when continuity matters — we'll rebuild the local provider network around your new address rather than hand you over mid-move.
