NDIS support coordination in Brisbane.
NDIS support coordination in Brisbane means a coordinator who visits you at home, knows which Brisbane providers currently have capacity, and can move quickly after a hospital discharge. The Care Coordinators are based in Redland City and cover the inner city, north, south, bayside, Logan and Ipswich — funded from Capacity Building in your plan, at no out-of-pocket cost. Referrals are answered within two business days.
A Brisbane coordinator who will sit at your kitchen table, knows which local providers actually have capacity this month, and can be at the hospital for a discharge meeting when it counts.
Where we visit
We work across greater Brisbane and cover Ipswich and Logan on the same visit runs, which means less waiting for an appointment and fewer "we'll call you instead".

— Local knowledge
What a Brisbane coordinator actually adds.
Who has capacity, this month
A provider list is useless if everyone on it is full. We keep asking, so we can tell you who can actually start — north side versus south side included.
Hospital discharge
We work to the discharge date with the social worker: in-home supports, equipment, and somewhere appropriate to go if home isn't ready.
Transport that works
Getting to appointments in Brisbane is its own problem. We factor in accessible transport, taxi subsidy and travel funding before we book anything.
Housing, honestly
If SIL is on the table we visit the houses with you and tell you what we saw — including when the answer is "not this one".
Brisbane questions
Which Brisbane suburbs do you visit?
Greater Brisbane — the CBD and inner north, Chermside and Nundah, Carindale and the bayside, Mount Gravatt, Sunnybank, Inala, and the northern corridor through North Lakes and Redcliffe. Ipswich and Logan are covered too.
Can you help with a hospital discharge?
Yes — and call as early in the admission as you can. We join planning meetings with the hospital social worker, work to the discharge date, and line up supports, equipment and housing options before the bed is needed.
How quickly can we start?
Capacity moves week to week, so we'll tell you honestly on the first call. If we can't start when you need us, we say so rather than parking you on a waitlist.
