Anyone can make a referral to The Care Coordinators — participants, family members, carers, support workers, hospital staff or other providers. Call 0490 695 228 or send the form below. We answer referrals within two business days and can usually meet within a week, in person across South East Queensland. The first conversation is free and there is no obligation.
What you can expect from us

- A named coordinator — not a call centre
- No waitlist at the moment
- Hospital discharge support at short notice
- After-hours contact for participants we work with
- An interpreter arranged on request
