We got tired of watching people give up on their own plans.

The Care Coordinators exists to help people navigate the NDIS with confidence and clarity.

Through personalised support coordination, recovery coaching and strong community partnerships, we help participants understand their plans, connect with the right services and work towards the goals that matter to them. Our approach is built on respect, inclusivity and genuine collaboration — support shaped around one person's circumstances, not a template.

By simplifying complex systems and advocating alongside the people we work with, we help build independence, stronger support networks and fuller lives in the community.

We come to you

Home visits, not just phone calls. Relationships are built face to face — with the participant, not only about them.

Plain words, always

If we use an acronym without explaining it, pull us up on it. Every document we send can come in easy English.

Your choice, your control

We present options and the trade-offs. You decide. You can change providers — including us — at any time.

Waitlists get chased, agreements get finished, reports get written on time for your plan review.

The Care Coordinators is run by Nick Evans and Jacquie Saunderson, from Redland City in bayside Brisbane. You deal with a coordinator directly — never a call centre — and you'll know whose name is on your plan from the first conversation.
